Job summary

Together for Youth in Buffalo, NY is seeking a Family Specialist to conduct strengths assessments, develop comprehensive treatment and discharge plans, and coordinate services for at-risk youth and their families in home and community settings.

Qualifications

• Bachelor's degree in human services or a related field.
• Minimum 1 year of experience working with at-risk youth and families and in case management.
• Flexible schedule with day, evening, and on-call hours.
• Valid driver's license with reliable vehicle; travel required.
• Ability to work with economically and culturally diverse populations.
• Bilingual ability is a plus.

Responsibilities

• Assess family strengths and needs and apply treatment modalities to support growth.
• Develop and evaluate comprehensive treatment and discharge plans.
• Deliver case management services and ensure required examinations.
• Conduct home and community visits at least twice monthly to support goals.
• Maintain relationships with schools and community agencies to support education.
