Job summary

Center for Humans Services seeks a Family Advocate to provide in-home and community-based case management for families connected to DR, AfterCare, and FFPSA programs. You will coordinate services across mental health, child welfare, and public benefits to promote family stability and safety.

The role requires bilingual English/Spanish communication skills, strong documentation, and the ability to work with diverse populations in California.

Qualifications

• Bachelor’s degree in social services or related field is required.
• Master’s degree is preferred.
• Fluent in English and Spanish for client interactions and documentation.
• Experience with trauma-impacted families and child welfare systems is valued.

Responsibilities

• Serve as primary contact for families referred to DR, AfterCare, and FFPSA programs.
• Provide intensive case management for families with complex needs.
• Conduct comprehensive assessments including PFS and ASQ-3 screenings.
• Coordinate Family Team Meetings and ensure family voice and choice.
• Develop and monitor Family Action Plans aligned with FFPSA goals.
• Assist families in navigating services across systems (mental health, housing, benefits).
• Conduct home visits and outreach to ensure follow-through on plans.
• Deliver workshops and trainings to strengthen protective factors.
