Job summary

Health Connect America seeks a Family Centered Treatment Specialist to deliver FCT services to children and families in home and community settings, aligning with the Family Centered Treatment model and fidelity requirements.

Qualifications

• Minimum of a Bachelor's Degree in human services or related field is required.
• Two years' experience working with youth or families.
• Current Family Centered Treatment Certification is preferred.
• Mississippi – Must be provisionally credentialed as a PCSS in Mississippi with eligibility to be fully credentialed as a CCSS in Mississippi.
• North Carolina – Must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis.
• Ohio – LSW or eligibility for provisional licensure preferred.
• Virginia – Current QMHP or ability to register as QMHP with the Virginia Board of Counseling.

Responsibilities

• Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
• FCT Certification is required and must be completed within one year of employment to continue providing FCT services.
• Complete FCT Checkoffs and other trainings within designated deadlines via the e-learning system.
• Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
• Provide 24/7 on-call coverage to a caseload of families and children, with a flexible work schedule.
• Complete Clinical Records and FCT documents with the child and family, uploaded timely in the EHR.
• Develop Service Plan in coordination with Treatment Team and maintain plan per authorization processes.
• Conduct ongoing revisions of Service Plan and implementation of the plan.
• Provide psychoeducation as indicated in the Service Plan.
• Conduct interventions with the family to move through the four phases of treatment and complete fidelity documents.
• Assist families to establish weekly goals and steps to reach them.
• Make referrals to services and resources for families and children.
• Develop and coordinate discharge plan and aftercare needs.
• Coordinate initial and ongoing assessment activities.
• Convene the Treatment Team for Service Plan.
• Consult with medical and non-medical providers and engage supports in person-centered planning.
• Ensure linkage to clinically appropriate services including evaluations.
• Provide and coordinate behavioral health services with other professionals.
• Monitor and document the consumer's progress.
• Participate in QA/QI activities including audits and improvement plans.
