Job summary

The Groden Center in Providence, RI seeks an Early Intervention Service Coordinator (Level II) to oversee and deliver services to infants and toddlers with developmental delays or disabilities. This full-time professional role focuses on coordinating services, documenting care, and engaging families in home and community settings.

Responsibilities include assessments, service plan development, team collaboration, and supporting transitions at age three.

Qualifications

• Experience coordinating early intervention services for infants and toddlers.
• Ability to work with families in home and community settings.
• Knowledge of service plan development and multidisciplinary team processes.
• Eligibility assessments for birth-to-three programs and transition planning.

Responsibilities

• Coordinate and provide EI services for assigned caseload.
• Work with children and families at home and in community settings daily.
• Document services and participate in EI team meetings/training.
• Contribute to development of individual service plans for child and family.
• Assist in transition planning to local education agency at age three.
