Job summary

A non-profit organization in Evergreen Park is seeking an entry-level Service Coordinator to support families of infants and toddlers with developmental delays. Qualifications

• Knowledge of typical/atypical child development.
• Obtain EI Service Coordination DHS credential.
• Valid driver’s license, auto insurance, and reliable vehicle.

Responsibilities

• Receive referrals and maintain Early Intervention case records.
• Provide information about Early Intervention services.
• Conduct intake activities for assigned families.
• Coordinate evaluations for Individualized Family Service Plans (IFSP).
• Monitor implementation and review of IFSPs.
