Job summary

Community Counseling Center seeks an Early Diversion Outreach Specialist to connect adults with mental illness and co-occurring substance use disorders to timely behavioral health supports.

You will engage with law enforcement, EMS, hospitals and community partners, conduct screenings, provide motivational support, and coordinate crisis stabilization and referral pathways. Brand-new positions emphasize outreach, care coordination, and community education.

Qualifications

• Bachelor's degree, or any four-year combination of higher education and qualifying experience, or four years of qualifying experience.
• Must hold or obtain by start date MO Class E Driver's License (Class D for IL Residents), and proof of continuous Auto Insurance.

Responsibilities

• Conduct community outreach and engagement to identify adults with mental illness and co-occurring substance use disorders at risk of criminal justice involvement.
• Build collaborative relationships with law enforcement, EMS, fire departments, hospitals, behavioral health providers, courts and other partners to improve referral pathways.
• Screen individuals for behavioral health needs and assist with early identification of mental health and substance use concerns.
• Provide brief engagement, motivational support, and crisis stabilization services to encourage participation in treatment and recovery.
• Connect individuals with behavioral health, substance use treatment, healthcare, housing, employment and other supports based on need.
• Maintain follow-up and navigation services until linked to long-term treatment, generally lasting up to three months.
• Educate individuals, families and partners regarding available behavioral health services and recovery resources.
• Assist with community education and training on behavioral health, substance use, suicide prevention, trauma-informed care and crisis intervention.
• Participate in system mapping, coalition meetings and planning to strengthen early diversion initiatives.
• When appropriate, respond with law enforcement and EMS during behavioral health calls for service.
• Document outreach activities, referrals and progress in EHR and grant-required systems accurately and confidentially.
• Collect and report program data to support grant compliance and program evaluation.
• Attend supervision, staff meetings and ongoing training as required.
• Perform other duties to support the Early Diversion Program and Community Counseling Center.
