Job summary

Community Counseling Center in Cape Girardeau, MO is hiring an Early Diversion Outreach Specialist to perform outreach, engagement, and care coordination for adults with mental illness and co-occurring substance use disorders. This role focuses on connecting individuals to appropriate behavioral health services and supports to reduce crisis involvement.

The position involves collaborative work with local agencies, screening for needs, and ensuring smooth referrals.

Qualifications

• Bachelor's degree or four-year combination of education and experience required.
• No less than four years of qualifying experience preferred.
• Must obtain MO Class E Driver's License by start date (Class D for IL residents).

Responsibilities

• Conduct community outreach to identify adults with mental illness and co-occurring substance use disorders at risk of criminal justice involvement.
• Build relationships with law enforcement, EMS, hospitals, and behavioral health partners to promote early diversion and referrals.
• Screen individuals for behavioral health needs and assist with early identification of concerns.
• Provide brief engagement and crisis stabilization to encourage treatment participation.
• Coordinate care connections to behavioral health, housing, employment, and other supports.
