Hope Network is seeking a Direct Support Professional (DSP) for the 2nd shift on-site in Michigan. This full-time role involves helping individuals achieve independence, participate in community activities, and access daily living supports.

You will work closely with care teams to monitor health, safety, and progress, documenting care plans and aiding in transportation and community engagement.

Formación

• High school diploma or GED required.
• Valid Michigan driver’s license.
• Ability to lift 50 lbs.
• CPR/First Aid certification required.

Responsabilidades

• Monitor and ensure safety 24/7 with various check patterns.
• Support crisis prevention, intervention, and resolution.
• Establish trusting relationships through consistent communication.
• Foster partnerships with individuals served, families, and professionals.
• Assist with activities of daily living and community participation.
• Document care plans and relay information between teams.
• Assist with transportation to activities and appointments.
• Promote positive behavior and independence.
