Job summary

Challenge Unlimited, Inc. seeks a Community Day Services Manager/QIDP to provide case management and person-centered services for individuals with disabilities. This role develops service plans, monitors progress, and ensures regulatory compliance to support client independence.

You will coordinate with families and staff to promote community integration, vocational activities, and skill development, while maintaining documentation and quality standards.

Qualifications

• Bachelor's degree in a human services-related field or RN.
• 1+ years of experience with people with Developmental Disabilities; IL QIDP eligibility.
• CPR, First Aid, CPI, QIDP and Direct Support Professional (DSP) training provided and completed within 4 months.

Responsibilities

• Promote person-centered services while providing high-quality care in a community-based setting for individuals with disabilities, coordinating with clients, families and guardians.
• Develop person-centered programming, evaluate progress, and maintain documentation and plans.
• Supervise facility-based and community-based services, training and integration opportunities.
• Transport clients for community outings, employment or appointments using company or personal vehicle.
• Assist with hiring, training, and empowering of team members; reinforce policies and procedures and ensure regulatory compliance.
