Job summary

Challenge Unlimited, Inc. seeks a Community Day Services Manager/QIDP in Alton, IL to provide case management and person-centered services for individuals with disabilities.

You will develop individualized service plans, monitor progress, and support independent living and community integration. The role requires coordination with clients, families, and providers, maintaining regulatory compliance, and supervising both facility and community-based services.

Qualifications

• CPR, 1st Aid, CPI, QIDP and Direct Support Person training provided by the company must be completed within the first 4 months.
• Proficient with Microsoft Word, Excel and Outlook.
• Must pass state and federal background checks and have a valid driver's license.

Responsibilities

• Promote person-centered services and coordinate with clients, families, and guardians to meet goals.
• Develop and update person-centered plans, monitor progress and document outcomes.
• Supervise facility and community-based services, provide training and transportation arrangements as needed.
• Assist with hiring, training, and mentoring of team members to ensure program success.
• Communicate regularly with staff about client updates and workplace changes.

Education

Bachelors degree in a human services-related field or Registered Nurse
