Job summary

Evergreen Treatment Services (ETS) seeks a Case Manager – LEAD for the REACH team at the Belltown site. This role delivers intensive case management to individuals affected by homelessness and co-occurring disorders, coordinating with LEAD partners and housing providers to address substance use and housing needs.

You will engage clients on the street and in service sites, develop individualized service plans, and advocate for clients within the criminal justice system, contributing to community

Qualifications

• Experience applying harm reduction principles in outreach and case management.
• Ability to engage participants in crisis and non-crisis settings with sensitivity.

Responsibilities

• Engage participants on the street and with service providers to establish relationships and offer services.
• Assess housing status and needs for medical, mental health, and other supports.
• Develop individualized service plans addressing food, shelter, health care and substance use treatment goals.
• Advocate for participants within the criminal justice system and coordinate with LEAD partners and housing providers.
