Community Council is hiring a Case Manager to provide direct service to adults with serious mental illness and their families, aiming to promote independence and reduce crises. You will access, coordinate, and monitor services to support stability in housing, relationships, and education.
The ideal candidate has a bachelor’s degree in a related field, at least 1 year of direct human service experience, and a valid driver’s license with an operable automobile. Full-time; Monday–Friday.
About the Organization Our services are designed to be holistic, geographically accessible, culturally sensitive, and coordinated with other related service systems, both formal and informal. Community Council has a long history of providing mental health services to Philadelphia residents and is one of the City of Philadelphia's original 12 Base Service Units. We provide a range of mental health and substance abuse direct services and case management. The organization has a diverse staff of 200 that deliver evidenced based services in a culturally competent manner.

The Case Manager is to provide direct service to adults who have a serious mental illness or emotional disorder and their families. The primary goals of the Case Manager are to promote as much independent living in the community as possible and reduce the chance of psychiatric emergency service contacts and hospitalizations. The Case Manager is to access, coordinate and monitor the resources and services available to the consumer and their family. In addition the case manager should provide support, training and assistance required for a safe and healthy community life promoted through stability in relationships, housing and education.
