A community-driven healthcare organization seeks a dedicated Case Manager to provide community-based case management services and support individuals affected by mental illness and substance use. The role includes developing Person Centered Plans, facilitating group meetings, and coordinating care with healthcare providers. Candidates must have a Bachelor’s degree in a relevant field and be a Qualified Mental Health Professional. Formación

• Must be a Qualified Mental Health Professional (QMHP).
• Valid and unrestricted Driver’s License required.
• Experience in mental health services or related fieldwork.

Responsabilidades

• Develop and implement Person Centered Plans for individuals.
• Facilitate weekly groups for individuals served.
