Job summary

Gulf Coast JFCS is seeking a Case Manager in Florida to provide direct case management for children, birth and foster families, and primary caregivers. The role focuses on assessment, planning, coordination, and advocacy to ensure services and continuity of care in resolving or preventing abuse or neglect.

You will maintain strong collaboration with all parties, participate in on-call rotations, and uphold confidentiality and compliance with laws and program policies.

Qualifications

• Bachelor's degree in a related field or an Associate's with two years' experience.
• Must have reliable transportation and a valid driver's license with auto insurance.
• Ability to transport clients as needed and maintain confidentiality.

Responsibilities

• Provide direct case management for children, birth and foster families and primary caregivers.
• Assess, plan, coordinate, and advocate for services and continuity of care.
• Maintain face-to-face contact and ongoing communication with all parties.
• Attend court dates, staffings, and required meetings with documentation.
• Provide transportation and supervision for children and participants.
• Maintain client confidentiality per policy and law.
• Participate in on-call rotation.
