Qualifications

• Graduate of an accredited RN program with active licenses in MA and NH.
• 3–5 years of clinical experience; CDI, risk adjustment, coding, or population health preferred.
• Strong knowledge of risk adjustment (HCC) and chronic condition documentation.

Responsibilities

• Perform chart reviews (prospective, concurrent, retrospective) to identify coding opportunities and risk gaps.
• Identify and validate documentation for risk-adjusted diagnoses and MEAT criteria.
• Support pre-visit planning and workflows to surface suspect conditions and improve risk capture.
• Educate providers and care teams on documentation improvement and coding guidelines.
• Conduct audits to validate accuracy and compliance, and provide feedback to coding teams.
