You will collaborate with CDI, quality, and population health leaders to support chronic disease documentation and value-based care initiatives. Ideal candidates have RN licensure in MA/NH, 3–5 years clinical experience, and strong knowledge of HCC risk

Qualifications

• RN licensed in MA and NH (per requirements)
• 3–5 years clinical experience; CDI/risk adjustment experience preferred
• Strong knowledge of risk adjustment, chronic condition documentation, and coding guidelines

Responsibilities

• Conduct chart reviews and abstraction to identify risk adjustment opportunities
• Support provider education and performance improvement on risk adjustment
• Identify missed chronic conditions and validate diagnoses using MEAT criteria
• Collaborate with CDI, coding, quality, and provider teams to ensure accurate patient representation
• Perform audits to validate accuracy and completeness of coding outputs
• Assist with pre-visit planning to surface suspect conditions and close risk gaps
• Contribute to scalable coding workflows across a large provider network
• Provide clinical guidance to providers and teams on documentation improvement
