Job summary

Comunilife in the Bronx, NY seeks an experienced Case Manager for its transitional residential program Safe Haven II. You will provide comprehensive case management, coordinate services, and support clients and families toward stable housing and well-being.

The role requires a Bachelor's (or Associate with equivalent experience) and at least two years in field work, with strong counseling skills and bi-lingual Spanish a plus.

Qualifications

• Minimum two years of field work experience in case management.
• Bachelors Degree required; Associates degree with commensurate experience will be considered.
• Experience with mentally ill populations and homeless services is preferred.

Responsibilities

• Provide comprehensive case management for assigned clients and families.
• Coordinate services, complete assessments and service plans.
• Conduct home visits and weekly client contacts as required.
• Connect clients to medical, mental health, and housing resources.
