Job summary

Comunilife in Bronx, NY is seeking experienced Case Managers for the Dona Rosita I Program. The role involves assessing resident needs, developing case plans, and linking them to services inside the residence and community.

You will help residents develop skills for independent living in permanent housing and collaborate with families and program staff. Required are a 4-year degree or higher in human services or related field with relevant case management experience, plus strong communication

Qualifications

Bachelor's degree or higher required in human services or related field.
Experience in case management and working with underserved populations recommended.
Knowledge of relevant housing and social services guidelines is preferred.

Responsibilities

Coordinate program admission with HASA Case Management and Housing units.
Interview referrals within 24 hours of HASA referral.
Follow HASA procedures for referrals and record-keeping.
Arrange and coordinate resident moves as approved by HASA and the program.
Assist residents with accessing Social Security, Medicaid, and other entitlements.
