Job summary

Comunilife Inc in the Bronx is seeking experienced Case Managers for the El Rio Residence Program. You will provide supportive services in a housing setting, helping clients define personal, vocational, and educational goals using a person-centered, harm-reduction approach.

Responsibilities include assessments, service planning, weekly contacts, linkage to medical care, housing coordination, advocacy, and collaboration with providers.

Qualifications

• Bachelor's degree required; minimum two years field work experience.
• Experience with mental illness populations.
• Preference for experience with homeless individuals in congregate programs; bi-lingual in Spanish a plus.

Responsibilities

• Provide supportive case management services in a housing setting.
• Conduct comprehensive assessments and develop service plans with short/long-term goals.
• Perform face-to-face contacts (home or office) weekly as per plan; coordinate with medical, mental health, and social services.
• Provide crisis intervention as needed and advocate for tenants' rights and housing stability.
• Collaborate with vocational counselors and entitlement specialists to support education and employment goals.
