Qualifications

• 2+ years of experience in Human Services/Case Management.
• Experience with high-service needs populations preferred.
• Strong computer skills, especially in MS and Google applications.

Responsibilities

• Manage a caseload of clients and monitor their progress.
• Coordinate access to referrals for Mental Health and Substance Recovery services.
• Provide crisis intervention to clients.
