Qualifications

• Strong written and verbal communication; proficiency with case notes and reports.

Responsibilities

• Maintain a caseload and provide regular case management services.
• Maintain resident records in compliance with agency standards.
• Conduct assessments and develop individualized Service Plans.
• Assist residents with benefits, healthcare linkage, and housing stability.
• Provide counseling, crisis intervention, and staff education as needed.
• Coordinate with community services including hospitals, HIV/AIDS programs, and mental health services.
• Attend ongoing training and assist with HASA reporting.
