Qualifications

• CCM or ACM certification required or obtained within two years of entry.
• 2 years rehabilitation experience preferred.
• Bachelor's degree required for non-nursing professionals.

Responsibilities

• Collaborate with an interdisciplinary team to plan patient care.
• Coordinate discharge planning and contingency plans.
• Monitor patient experience and care quality.
• Complete case management addenda and required documentation.
• Ensure regulatory compliance and payor requirements.
• Participate in utilization review and ACT meetings.
