Job summary

The Case Manager – Scattered Site at Open Arms Housing in Washington, DC, provides case management for single adult women in the PSHP program. You will coordinate services, develop service plans, and advocate for housing stability using Housing First and Harm Reduction approaches.

Requirements include an MA in Social Work or related field, 2 years of direct case management, experience with chronically homeless adults or substance use, and strong communication and organizational skills.

Qualifications

  • MA degree in Social Work, Psychology, or related fields.
  • Experience working with adults with serious mental illness, chronically homeless adults, and/or adults with substance use.
  • Two years of direct case management experience.
  • Excellent written and oral communication skills.
  • Strong interpersonal and organizational skills.
  • Ability to respond to crisis situations.
  • Strong computer proficiency with Microsoft Office Suite, Box, and Case-Management Software.

Responsibilities

  • Support clients moving into stable long-term housing.
  • Advocate for and assist residents in obtaining services (health, mental health, housing referrals, financial assistance, etc.).
  • Meet with residents to assess needs and develop service plans.
  • Coordinate services using Housing First and Harm Reduction approaches.
  • Maintain case notes and records in compliance with policy.
  • Attend staff meetings and trainings.
  • Collaborate with ACT teams and other treatment teams in place.
