Job summary

Qualifications

• Bachelor's degree in social, behavioral or human services.
• At least one year of professionally supervised work in mental health, chemical dependency or social services.
• Valid driver’s license with fewer than six points, proof of insurance, and an operational vehicle.

Responsibilities

• Initiates services with and engages formally homeless, disabled individuals in the community under a Supportive Housing program.
• Develops goals and objectives for clients to maintain housing and meet individual goals, including mental health treatment, substance use issues, health care linkage, income growth, and self-sufficiency.
• Completes and maintains progress notes and documentation according to agency and regulatory service standards.
