Job summary

Lutheran Family and Children’s Services (LFCS) in Kansas City, MO is seeking a Case Manager/Social Worker who will coordinate and provide client-centered care to vulnerable families and children.

You will conduct case management, pregnancy counseling, parenting and adoption education, and community outreach; develop service plans with clients and advocate for their goals within program protocols.

Qualifications

• Bachelor's or Master's Degree in Social Work, Counseling, Psychology, Criminal Justice, or other Human Behavior Science.

Responsibilities

• Coordinate and provide care to clients that are safe, timely, effective, efficient, equitable, and client-centered.
• Provide case management, pregnancy counseling, parenting and adoption education, community education and outreach, and post-adoption support for birth parents.
• Create a service plan in conjunction with the client to develop goals and objectives that are consistent with what the client wishes to accomplish.
• Counsel, advocate, and teach clients the skills identified in their service plan face to face in home or virtually.
• Respond to client’s crisis situations within program parameters in a timely manner according to program protocols.
