Job summary

Catholic Charities Brooklyn and Queens, Inc. is seeking a Case Manager II to deliver comprehensive case management and crisis intervention services for clients at risk of homelessness and those with prior shelter history.

You will perform intake, assessment, and long-term planning to help stabilize households. The role requires a BS in Human Services or related field, with at least one year in social services, and flexibility including evenings/weekends.

Qualifications

• BS degree in Human Services or related field.
• One year of experience in a social service setting or with individuals/families.
• Bilingual Spanish a plus.
• Ability to work flexible hours including weekends/evenings as needed.
• Able to conduct home/field visits and travel across the five boroughs.
• Frequently lifts/moves up to 10 pounds.

Responsibilities

• Provide case management, assessment and intake to determine eligibility.
• Ensure regular visits/contact for each assigned case.
• Maintain case records and input data into databases.
• Assess needs of the whole consumer/family including basic needs.
• Follow-up on referrals made on behalf of a consumer/family.
