Qualifications

• One year of experience in a social service setting or providing direct services.
• Ability to conduct home/field visits and travel throughout the five boroughs.

Responsibilities

• Provide case management services and assess participant eligibility.
• Maintain case records and ensure information is inputted into databases.
• Follow up on referrals and respond to all inquiries.
