Job summary

Catholic Charities Brooklyn and Queens, Inc. seeks a Case Manager II to support clients at risk of homelessness through comprehensive case management, crisis intervention, and referrals.

Under supervision, you will assess needs, complete intake, and coordinate services. Responsibilities include maintaining case records, arranging housing and financial assistance, and making home visits across the five boroughs.

Qualifications

• BS degree in Human Services or related field.
• One year of social services or direct client services experience.
• Bilingual Spanish skills are a plus.
• Ability to work flexible hours, including weekends and evenings.
• Willingness to conduct home/field visits and travel across the five boroughs.
• Ability to lift/move up to 10 pounds.

Responsibilities

• Provide case management, assessment and intake to determine service eligibility.
• Conduct regular visits and maintain contact with participants.
• Maintain case records and input information into databases.
• Assess needs of the client and family including basic needs like food, shelter.
• Follow-up on referrals and respond to referrals made to the program.
