Qualifications

• One year experience in a social service setting or in providing direct services to individuals or families.
• Ability to work flexible hours, including weekends and evenings.
• Able to conduct home/field visits and travel throughout the five boroughs as needed.

Responsibilities

• Provide case management services and assess participants for eligibility.
• Ensure regular case visits and maintain accurate case records.
• Assess client needs including food, clothing, and shelter.
