Job summary

Nation’s Finest in the United States is seeking a full-time Case Manager to support participants in achieving stability, wellbeing, and self-sufficiency. The role provides client-centered case management, outreach, housing navigation, and employment support.

You will connect clients with services, maintain confidential records, coordinate with partners, and help with budgeting and housing resources. A bachelor’s degree and experience in social services are preferred; veterans status desirable.

Qualifications

• Minimum 1 year of experience in case management or related field.
• Bachelor’s degree in social work or related field preferred.
• Knowledge of social services, housing programs, and crisis intervention.
• Proficiency with EHR and client tracking systems.
• Commitment to trauma-informed, client-centered practices.

Responsibilities

• Conduct client screenings, intakes, and assessments to determine needs.
• Develop, implement, and monitor service plans with clients.
• Coordinate supportive services including financial assistance and health benefits.
• Educate clients on program expectations and rules.
• Engage in outreach to connect clients with resources.
• Build relationships with landlords and community agencies.
