Job summary

PATH is hiring a Case Manager for the Zephyr PSH site in San Diego, CA. This role supports clients through housing transitions using a flexible, whatever it takes approach, coordinating ISP goals with services and income supports.

The role emphasizes ethical practice, strong data interpretation, and collaboration with partners to address housing, health, and vocational needs. On-site work is expected with a focus on PATH’s mission.

Qualifications

• Two years of experience in healthcare, case management, homeless services or related field.
• All levels of education and experience are welcome.
• Must meet mandatory requirements including eligibility to work in the U.S., CPR/First Aid training, and screenings.

Responsibilities

• Complete intake and develop ISP addressing housing goals and supports.
• Provide referrals and linkages to resources to meet ISP goals.
• Complete program assessments (e.g., VI-SPDAT).
• Evaluate progress with 90-day reviews and adjust plans.
