Job summary

Hesed House’s LIGHT-House Program seeks a Case Manager to provide ongoing supportive case management for individuals and families transitioning from homelessness toward stability. You will coordinate services, guide participants toward housing, employment, health, and independence, and connect them with community resources to promote long-term success.

Responsibilities include goal setting, maintaining records, and serving as an advocate within a supportive housing framework.

Qualifications

• Bachelor’s degree in human services or a related field is required.
• Experience with homeless or vulnerable populations preferred.
• Bilingual English/Spanish is a plus.
• Must pass DCFS and criminal background checks.
• Must have valid driver’s license.

Responsibilities

• Provide ongoing case management and goal setting for housing, employment, health, and self-sufficiency.
• Coordinate services and connect participants to community resources.
• Maintain records and progress notes; document using SP5.
• Assist in unit preparation, move furniture, and arrange transportation as needed.
• Engage participants in community activities and life-skills training.
