Job summary

Metropolitan Public Defender Service Inc in Hillsboro, OR seeks a Case Manager to join our Washington County Office. You will engage clients to identify resources, coordinate care, and develop discharge plans for those released from incarceration or hospitalization.

The role requires collaboration with attorneys and staff to ensure clients access housing, healthcare, public benefits, and other services. Travel may be required.

Qualifications

• Minimum of two years of experience in case management or an associate’s degree in a social-service related field.
• Training and experience with Trauma-Informed Care.
• Familiarity with substance use disorders and the recovery process.
• Familiarity with the criminal justice system.
• Excellent verbal and written communication skills.
• Ability to travel to and work within criminal justice system facilities and to testify in court without impeachment.

Responsibilities

• Engage with clients to identify and refer to desired community resources (housing, treatment, benefits, etc.).
• Partner with attorneys and staff to create discharge plans for clients released from incarceration or hospitalization.
• Maintain contact with clients throughout the life of open cases and advocate for their needs.
• Provide care coordination and systems navigation to connect clients to services.
