Qualifications

• Two years of case management or related social-service work experience.
• Training/experience with Trauma-Informed Care.
• Familiarity with substance use disorders and recovery processes.
• Familiarity with the criminal justice system; strong communication skills.

Responsibilities

• Identify and refer clients to community resources (housing, health care, public benefits).
• Develop discharge plans with attorneys and staff for clients leaving incarceration or hospitalization.
• Maintain contact with clients to advocate for services throughout active cases.
