Misericordia Heart of Mercy seeks a Case Manager/QIDP to work with residents, families, and an interdisciplinary team to implement personalized programs that promote independence and well-being.

You will oversee goals, support skill development, and advocate for residents while collaborating with nurses, DSPs, CNAs, therapists, and social workers. A bachelor's degree and 1+ year of developmental disability experience are required.

Qualifikationen

• Bachelor's degree required, related fields preferred (Psychology, Sociology, Special Education, Rehabilitation)
• At least 1 year of experience with developmental disabilities
• Strong written and verbal communication skills
• Demonstrated leadership and teamwork

Aufgaben

• Oversee resident programs, goals, activities, and community outings
• Develop and implement individualized programs based on each resident's needs and goals
• Support residents in developing skills for greater independence
• Collaborate with interdisciplinary team to monitor and adjust programs
• Advocate for residents' rights and well-being
• Provide leadership to staff supporting resident programs
• Communicate effectively with residents, families, and professionals
• Manage time/resources and meet deadlines
• Provide physical care to residents as needed
