Job summary

Misericordia Heart of Mercy is seeking experienced Case Managers / QIDPs in Chicago, IL. You will work closely with residents, families, and an interdisciplinary team to build individualized programs and support residents toward greater independence.

Responsibilities include overseeing programs, coordinating services, and guiding residents to achieve personal goals while advocating for rights, dignity, and quality care. Competitive hourly pay with benefits.

Qualifications

• Bachelor's degree required in human services or related field.
• At least 1 year of experience with developmental disabilities.
• Strong written and verbal communication and leadership abilities.

Responsibilities

• As a QIDP, oversee resident programs, goals, and activities
• Develop individualized programs based on residents' needs and interests
• Support residents in developing independence and life skills
• Collaborate with interdisciplinary team to monitor and adjust plans
• Provide leadership to staff supporting resident programs
• Ensure residents receive quality care and advocacy for rights
