Misericordia Heart of Mercy is seeking a Case Manager/QIDP to coordinate individualized programs for residents with developmental disabilities. You will build relationships with residents, families, and an interdisciplinary team to support independence and personal goals.

Qualifications

• Bachelor's degree in human services or related field.
• At least 1 year of experience with developmental disabilities.
• Strong written and verbal communication skills.
• Demonstrated leadership ability.
• Ability to work as part of a team.
• Commitment to protecting and promoting the rights of residents.
• Strong organizational and time-management skills.
• Ability to supervise staff and manage resources.

Responsibilities

• Oversee resident programs, goals, activities, and community outings.
• Develop and implement individualized programs based on residents' needs, abilities, interests, and goals.
• Support residents in developing skills that promote greater independence.
• Collaborate with the interdisciplinary team to develop, monitor, and adjust resident programs.
• Provide leadership and direction to staff supporting resident programs.
• Communicate effectively with residents, families, coworkers, and other professionals.
