Job summary

The Salvation Army Great Lakes Division is seeking a Case Manager in Detroit to provide comprehensive case management, education, and supportive services. You will work with clinical staff to coordinate access to services and support aftercare planning.

You will maintain accurate records, participate in clinical reviews, attend staff meetings, and pursue ongoing professional development to enhance treatment skills.

Qualifications

• Bachelor's degree in social work or equivalent required.
• Experience in case management preferred.
• Experience with mental health and/or substance abuse populations preferred.

Responsibilities

• Conduct didactics/educational lectures.
• Advise clients on risk prevention and available community resources.
• Establish trusting relationships with patients and families and provide support.
• Assist clients in accessing treatment services and addressing barriers.
• Provide clinical services to screen, assess and determine level of care.
• Maintain case records per Harbor Light standards.
