Job summary

Olympic View Post Acute in Port Angeles, WA seeks a Case Manager (Social Work) to coordinate discharge plans, advocate for patients, and connect families with resources from admission through home. You will work with an interdisciplinary team, conduct Social Service Assessments within 7 days, manage EMR notes, and support patients in navigating Medicare, Medicaid, VA benefits and other services.

Qualifications

• Bachelor's degree in Social Work or Sociology (MSW preferred).
• Minimum 2 years of case management or social work experience.
• Strong communication and empathetic approach with patients and families.
• Comfort working in an EMR and managing multiple priorities on deadline.

Responsibilities

• Own case management, utilization review, and discharge planning from admission to discharge.
• Be the go-to contact for patients and families during the stay.
• Lead or support rounds and care plan conferences to review goals and post-discharge needs.
• Coordinate discharges: placement, home health, transportation, and adaptive equipment.
• Review service utilization and prepare insurance denial letters when appropriate.
• Navigate Medicare, Medicaid (Apple Health), Social Security, VA benefits, and other resources.
• Complete Social Service Assessments within 7 days of admission and maintain EMR progress notes.
• Document social service needs with measurable goals in each care plan.
• Support informed healthcare decisions, including advance directives.
• Refer complex cases for counseling and connect with outside services.
• Assist with admissions: tours, paperwork, and facility orientation.
• Help resolve grievances and support resident/family council meetings.
