Olympic View Post Acute in Port Angeles, WA seeks a Case Manager (Social Work) to coordinate discharge plans, advocate for patients, and connect families with resources from admission through home. You will work with an interdisciplinary team, conduct Social Service Assessments within 7 days, manage EMR notes, and support patients in navigating Medicare, Medicaid, VA benefits and other services.

At Olympic View Post Acute, our Case Manager is the person patients and families lean on from the day they arrive to the day they leave. You'll coordinate discharge plans, advocate for patients, connect families with the resources they need, and make sure every plan holds up once someone gets home. Own case management, utilization review, and discharge planning for patients from admission through discharge
Meet each patient and family early, explain your role, and be their go-to contact throughout the stay
Lead or support interdisciplinary rounds and care plan conferences to review goals, resource use, and post-discharge needs
Coordinate discharges: placement, home health, transportation, and adaptive equipment
Review utilization of services and prepare insurance denial letters when appropriate
Help patients and families navigate Medicare, Medicaid (Apple Health), Social Security, VA benefits, and other financial or legal resources
Complete Social Service Assessments within 7 days of admission and keep progress notes current in the EMR
Document social service needs in each patient's care plan with clear, measurable goals
Support patients in making informed healthcare decisions, including advance directives
Refer complex cases for counseling and connect patients with outside services
Assist with admissions as needed, including tours, paperwork, and facility orientation
Help resolve grievances and support resident and family council meetings

Bachelor's degree in Social Work or Sociology (Master's in Social Work preferred)
At least 2 years of case management or social work experience
Strong communication skills and a genuine, empathetic approach with patients and families
Comfort working in an EMR and managing multiple priorities on deadline
Ability to pass a pre-employment physical, TB screening, drug screen, and background check
