Job summary

Olympic View Post Acute in Port Angeles, WA invites a dedicated Case Manager (Social Work) to guide patients from admission through discharge. You’ll coordinate discharge planning, advocate for patients, connect families with resources, and ensure plans hold after they leave for home.

An ideal candidate has a social work background with at least 2 years in case management or similar roles; a BS in Social Work or Sociology is required and MSW preferred, plus comfort with EMR and multi-priority

Qualifications

  • Bachelor's degree in Social Work or Sociology is required; MSW preferred.
  • At least 2 years of case management or social work experience.
  • Strong communication skills with patients and families.
  • Comfort working in an EMR and managing multiple priorities on deadline.
  • Ability to pass pre-employment physical, TB screening, drug screen, and background check.

Responsibilities

  • Own case management, utilization review, and discharge planning from admission through discharge.
  • Meet patients and families early, explain your role, and be their go-to contact.
  • Lead or support interdisciplinary rounds and care plan conferences.
  • Coordinate discharges: placement, home health, transportation, and adaptive equipment.
  • Review utilization of services and prepare denial letters when appropriate.
  • Help navigate Medicare, Medicaid, Social Security, VA benefits, and other resources.
  • Complete Social Service Assessments within 7 days of admission and maintain progress notes in the EMR.
  • Document social service needs with measurable goals in each care plan.
  • Support patients in informed healthcare decisions, including advance directives.
  • Refer complex cases for counseling and connect with outside services.
  • Assist with admissions including tours, paperwork, and orientation.
  • Help resolve grievances and support resident and family council meetings.

Case Manager (Social Work) – $35–$45/hr | Olympic View Post Acute | Port Angeles, WA
Help patients get home, and help them get there well.

At Olympic View Post Acute, our Case Manager is the person patients and families lean on from the day they arrive to the day they leave. You'll coordinate discharge plans, advocate for patients, connect families with the resources they need, and make sure every plan holds up once someone gets home. If you have a social work background and want to do meaningful work in a close-knit facility on the Olympic Peninsula, we'd love to meet you.

What You'll Do
  • Own case management, utilization review, and discharge planning for patients from admission through discharge

  • Coordinate discharges: placement, home health, transportation, and adaptive equipment

  • Help patients and families navigate Medicare, Medicaid (Apple Health), Social Security, VA benefits, and other financial or legal resources

  • Complete Social Service Assessments within 7 days of admission and keep progress notes current in the EMR

  • Support patients in making informed healthcare decisions, including advance directives

  • Refer complex cases for counseling and connect patients with outside services
