Job summary

Kaiser Permanente is hiring an RN Case Manager Liaison Nurse in Olympia, WA, focused on discharge planning and follow-up care. The role emphasizes collaborative care and patient advocacy within network facilities.

The position offers 0.6 FTE with a Mon-Fri schedule, 8:00 AM-4:30 PM, and every other weekend rotation, plus a $5,000 sign-on bonus for eligible external hires. Current WA RN license is required.

Qualifications

• ADN required.
• Current RN license in WA.
• Experienced level candidate.

Responsibilities

• Ensure patients referred to case management meet established criteria.
• Assess patients' physical, mental, financial, and psychosocial status.
• Develop patient-centered case management plans with specific goals and outcomes.
• Evaluate the ability of designated caregivers to provide support.
• Coordinate and implement interventions using evidence-based guidelines.
• Collaborate with healthcare professionals regarding the plan of care.
• Monitor and evaluate patient responses to therapeutic interventions.
• Facilitate referrals for home health care, long-term care, and hospice.
• Collect data to evaluate the effects of care coordination on quality outcomes.
• Assist patients in understanding and complying with their medical treatment plans.
