Qualifications

• Bachelor's Degree (or equivalent experience) in Rehabilitation, Human Services, Social Work, Psychology, or related field.
• Experience working with individuals with disabilities preferred
• Experience supporting medically fragile individuals is a plus
• Excellent communication and relationship-building skills
• Strong organizational and time-management abilities
• Ability to work independently while collaborating with a multidisciplinary team
• Valid Wisconsin driver's license
• Good driving record
• Reliable transportation and insurance meeting agency requirements

Responsibilities

• Manage an assigned caseload while developing meaningful relationships with participants.
• Develop, coordinate, and monitor IPPs (Individual Program Plans).
• Advocate for participant rights, choices, and personal goals.
• Coordinate services with families, guardians, residential providers, funding agencies, and community resources.
• Monitor participant progress and adjust supports as needs change.
• Provide guidance, encouragement, and problem-solving support.
• Maintain accurate electronic documentation and case records.
• Make referrals to community resources and additional RCS services.
• Collaborate with supervisors and interdisciplinary teams to ensure participant outcomes.
• Participate in continuous improvement efforts and agency initiatives.
• Help create an environment where participants are treated with dignity, respect, and compassion.
