Job summary

Optimal Case Management is hiring a Case Manager/Coordinator of Community Services (CCS) to develop person-centered supports for individuals with IDD and families. The role combines community-based work, home-based administrative tasks, and regional travel to coordinate services and advocate for needs.

You will build trusting relationships, translate goals into supports, monitor services, and ensure timely documentation in accordance with requirements.

Qualifications

• Bachelor’s or graduate degree in social work or related field.
• Supervised field experience in case management or two years direct clinical care.
• Strong written and verbal communication skills.

Responsibilities

• Develop relationships with individuals and families and coordinate services around their needs.
• Coordinate and monitor services across providers and community resources.
• Maintain timely, person-centered documentation and meet productivity expectations.
• Travel regularly within the assigned region to support clients and meetings.
