Job summary

Richmond, Indiana-based Damar Services, LLC is seeking a dedicated Case Manager to lead, direct and monitor treatment services for individuals with DD/ID or autism within our mission-driven program. You will manage all aspects of case management and promote agency philosophy in service delivery.

You will develop IPPs, coordinate with Residential Managers, lead program meetings, and ensure timely documentation, IPP updates, assessments, and family engagement across a caseload.

Qualifications

• Bachelor’s Degree in education, social work, or human services field (or equivalent experience).
• Minimum age 21 and valid Indiana driver’s license.
• Background check and pre-employment drug screen required.

Responsibilities

• Lead, direct and monitor treatment services for assigned clients.
• Develop and update Individualized Program Plans (IPPs) within 7 days of admission.
• Represent clients in meetings, court reviews, and IPP updates as needed.
• Maintain case notes, data sheets, and timely documentation of progress.
• Collaborate with Residential Managers to ensure program execution and safety.
• Engage families and coordinate referrals as part of caseload management.
