Qualifications

• Must have at least one year of experience treating DD/ID population or specialized training.
• Valid Indiana driver’s license and driving record per insurance requirements.
• Clear background, pre-employment drug screen and physical.
• Ability to write and update IPPs and service notes accurately and timely.

Responsibilities

• Represent clients at doctor appointments and exchange information accurately.
• Maintain service notes, data sheets, and data collection for assigned clients.
• Plan and lead Program meetings and develop IPPs within 7 days of admission.
• Collaborate with Residential Managers for unit programming execution.
• Direct IPP implementation and provide guidance to staff.
• Update IPPs monthly to ensure readiness by the 6th of each month.
• Complete ICAP assessments within 30 days of admission and Ansell Casey assessments as needed.
• Monitor IL goals, complete IL monthly reports, and ensure family engagement weekly.
