Job summary

Damar Services, Inc. in Richmond, IN is seeking a dedicated Case Manager to lead treatment services for individuals with DD/ID or autism. You will drive IPP development, coordinate with medical and residential staff, and ensure timely documentation of plans and goals.

Qualifications

• Must have at least one year of experience in treating DD/ID population or specialized training.
• Bachelor’s Degree in education, or Bachelor’s/Master’s in social work or related field.
• Three+ years of experience with DD/ID under supervision (or similar).
• Drivers license and clean driving record required.

Responsibilities

• Lead, direct and monitor treatment services and promote agency philosophy.
• Represent clients at doctor’s appointments and exchange information accurately.
• Develop and update IPPs within 7 days of admission and as needed.
• Plan and lead Program meetings and ensure IPP implementation.
• Maintain service notes, data sheets, and monthly IL-related reports.
• Coordinate with Residential Managers to ensure unit programming execution.
• Maintain communications with families and referral sources weekly.
