Qualifications

• Bachelor's degree required in social services or related field.
• Zero years of experience required with a Bachelor's degree.
• Knowledge of SCS programs and services, policy, and procedure.
• Familiarity with applicable regulations and licensure standards.
• Strong written and oral communication skills.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client's needs.
• Participates in development of client's service plan, consistent with assessment.
• Arranges for delivery of needed services, as identified in assessment.
• Assists client in accessing needed services through referrals or linkages.
• Monitors client's progress by making referrals, tracking appointments, performing follow-up on services rendered, and reassessments.
• Performs advocacy activities on behalf of client (e.g., writing letters to expedite entitlement payments).
• Prepares and maintains case records documenting contacts and services in accordance with standards.
• Provides case consultation with service providers and collaterals.
• Communicates with supervisor about case management issues and resource development.
• Provides psychosocial education to clients and/or family members.
• Performs crisis assistance including emergency referrals and coordination of services.
• May serve as Primary Case Coordinator (PCC) depending on skill and autonomy.
• All staff are expected to adhere to safety management and Joint Commission standards.
