Job summary

Qualifications

• Bachelor’s degree in a qualifying field is required.
• Completion of the state-required case management course within six months of employment.
• At least 1 year of full-time experience with adults in a human services setting is preferred if applicable.

Responsibilities

• Conducts and documents comprehensive assessment of client and/or client’s needs.
• Participates in development of client’s service plan, consistent with assessment.
• Arranges for delivery of needed services, as identified in assessment.
• Assists client in accessing needed services through referrals or linkages.
• Monitors client’s progress by tracking appointments, referrals, and follow-ups.
• Performs advocacy activities on behalf of client (e.g., letters to expedite entitlement payments).
• Prepares and maintains case records in accordance with standards.
• Provides case consultation with service providers.
• Communicates with supervisor about case management, resources, and new resources.
• Provides psychosocial education to clients and/or family members.
• Performs crisis assistance and makes emergency referrals as needed.
• May serve as Primary Case Coordinator (PCC) depending on skill and autonomy.
