Job summary

Lutheran Social Services of New York is seeking a dedicated Case Manager / Art Coordinator for the Bergen Street Residence in Brooklyn. This dual role supports residents’ wellness through advocacy, assessment, planning, and creative engagement.

You will coordinate art groups, link clients to health and housing resources, maintain confidential records, and collaborate with the Supervisor of Social Services to deliver compassionate, client-centered care.

Qualifications

• Bachelor's degree in social services and/or art is preferred.
• Minimum 2 years in human services field and experience with homelessness, HIV, substance abuse, mental illness, and entitlements.
• Ability to develop, organize, and facilitate art groups and art therapy.

Responsibilities

• Advocate for residents and meet with caseload clients weekly or as needed.
• Link clients with providers and resources across health and social services.
• Organize and facilitate on/off-site art groups and track participation.
• Maintain audit-ready client records and complete psychosocial assessments.
• Escort clients to scheduled psychiatric/medical care when needed.
